Via Early Intervention
Parental or Caregiver Responsibilities

Now HiringEarly Intervention is successful when there is a strong commitment from both therapists and families. Parents and caregivers can help make this partnership work be doing the following while enrolled in Via Early Intervention:

  1. Complete and return all required Via permission forms.

  2. Assist staff in obtaining an updated health form and Early Intervention prescription from child’s physician by providing up-to-date health care provider information.

  3. Be at home and ready for the staff person at the scheduled time.

  4. Be part of the session with your child and the Via staff person. Whether it is participating in activities or asking questions, involvement in your child’s program will mean greater success for your child.

  5. Supervise other children and pets in the house. Talk to the therapist about ways to include other children in the sessions.

  6. Notify the staff, as soon as possible, but a minimum of two hours in advance, if you need to cancel your appointment. See staff list for phone number and voicemail box numbers and pager or cell phone numbers.

  7. Protect your family, other families and Via staff by canceling services when there is illness in the household. Via staff may not provide services if any household member is running a fever, have thick/colored mucous discharge, diarrhea, or any contagious condition.

  8. Failure to notify staff of cancellations in advance three times in a row or a total of five times will result in a suspension of services. Services can not be resumed until a team meeting is held with the parent, county service coordinator and Via representative to determine a plan of action.

  9. Report any change of address or telephone number to Via staff members.
